Make sure " Multiple displays " is not set to " Disabled. Select apply once done for all displays. No luck. Sorry for the inconvenience you are facing. Have you installed any USB software installed? Is so uninstall the application and check if the 3 monitors work. You can also reduce the resolution of the 2 monitors and check if the 3rd one works. I have the same connections as your card.
